Upon arrival the patient is given the patient information sheet, notice of privacy act and receipt. The patient is asked to fill out the information and sign the notice of privacy act receipt.
Once the patient has completed this paperwork the new patient is retrieved from the lobby and taken back to an interview area where all the demographic and income information is verified and entered into the computer.

Copies are made of the patient’s identification and insurance cards.

If the patient is self-pay, copies of check stubs, W-2’s, and/or tax forms are verified or requested.

If the patient is insured a co-payment is collected.

The patient is made aware of their financial responsibility.

The patient is escorted to the waiting area and the chart is given to the appropriate nurse for the provider that they will be seeing.

The nurse will call the patient’s name and wait for a response, even if the patient is not checked into the practice management program.

When the patient responds, the nurse will escort the patient back to the pre-exam room to check their height and weight. Upon completion of height and weight the nurse will escort the patient to the exam room to complete the check in process.

After the exam has been completed the patient will be directed to a billing station so that the checkout process can begin

If the patient is insured they will be sent to a billing station where information will be verified and any further appointments can be made.

A self-pay patient will also be sent to an appropriate billing station where a bill will be generated for all service balances.

Self-pay or Insured Self-Pay patients may be eligible for discounted services based on the patient's household income. This is known as the Sliding Fee scale.
